RELEASE: The following must be signed by a parent or guardian before any student will be accepted to Debbie's School of Dance.

I understand that all the precautions have been and will be taken for the safety of my child/children. 

I understand that in the event of extreme weather, natural disasters, or government-mandated shutdowns we reserve the right to provide and will provide zoom classes.  Enrollment, payment, and withdrawal policies will apply.

I hereby release Debbie's School of Dance, Inc., and all representatives agents and/or employees from any liability for personal injury or death or damage or loss of property incurred because of the result of my child's participation in dance acrobatics and/or gymnastics, regardless of whether caused by the negligence of said parties or otherwise.

INSURANCE: I understand that Debbie's School of Dance, Inc., does not carry medical insurance for its students and that it is REQUIRED that all students be covered by their own family insurance policies and that if injury occurs it is understood that the student's family policy is my/our ONLY source of reimbursement.

I hereby give permission for my child to be transported to the nearest doctor or hospital in case of injury or emergency if unable to locate the parent or guardian.

PLACEMENT POLICIES:

Debbie's School of Dance reserves the right to place all students in dance classes at the level deemed appropriate by the Debbie's School of Dance Faculty.

NO parent placement of students will be allowed.

The Debbie's School of Dance Faculty takes into account the physical capabilities of each student and assigns the most beneficial class level.  To do otherwise is detrimental to a student's self-confidence and the progress of dance education.

Placement requests for higher-level classes because of student friendship/carpools will not be allowed.

If a student attends a class at an inappropriate level without Faculty approval, the student will be removed from the class and placed at the Faculty recommended level.

NEW STUDENTS: It is difficult to properly place all new students in the most beneficial classes until the season has begun and the new students have attended classes.  Debbie's School of Dance appreciates your patience.  The Faculty may recommend a more valuable class for your child once classes have begun.
